Surgical Repair of Scleral Ulcer with Bovine Pericardium as a Complication of Pterygium Excision - Two Case Report -

Abstract
The purpose of this study is to evaluate the effect of bovine pericardium used in scleral ulcer result from excision of pterygium. Possible causes of scleral ulcer after pterygium excision are application of topical mitomycin C, excessive episcleral cauterization, and excessive removal as far as episclera. Two eyes of 2 patients who had painful scleral ulcer resulting from excision of pterygium were grafted with bovine pericardium (Lyoplant¢ç). The free conjunctival flap was placed over the graft area. We measured the results and complications of treatment. One patient was showed loss of conjunctival injection, foreign body sensation, epiphora and there was good conjunctival healing after procedure. Another patient was also good treatment result until 3 weeks after graft, but to the next follow up period, was showed gradual ischemic loss of overlying conjunctival tissue and progression of scleral ulcer. The microorgnism culture was negative. The possible causes of recurrent scleral ulcer at this case seemed to be delayed wound healing and poor vascularization to sclera and conjunctiva in patient with diabetes mellitus, less likely to graft itself problem. So the authors suggest bovine pericardium appears to be an good substitute for autogenous or homologues tissue in scleral treatment
